Assessing the Rotator Cuff Muscles in Shoulder Pain

How often does a patient come to your clinic saying, “I have a rotator cuff injury”? Working with painful or injured rotator cuff muscles can be a daunting prospect. There are some 30 muscles involved with the shoulder joint. Here are a few other shoulder facts:

  • The shoulder is stabilized more by muscles (the rotator cuff) than by ligaments;
  • There are 17 muscles attached to the scapula alone. (If you want to count them, don’t forget the omohyoid);
  • The shoulder has direct muscle attachments along the spine from the occiput to the pelvis;
  • The only bony attachment between the shoulder complex and the axial skeleton is the sterno-clavicular joint.

Assessing the Rotator Cuff Muscles in Shoulder Pain

For a basically qualified Holistic Masseur, who may only have learnt a short shoulder sequence, it’s hard to know what to do. But a Sports Massage Therapist should have a much deeper knowledge of the structure and function of the shoulder joint. The first task is to differentiate the four individual muscles which make up the rotator cuff: infraspinatus, supraspinatus, subscapularis and teres minor. These four muscles are most important when it comes to stabilizing the shoulder and performing its actions. Unlike the hip, which is stabilised largely by ligaments, it’s these four muscles that hold the shoulder together. And because muscles are more prone to injury than ligaments, the shoulder is a more vulnerable joint.

Each rotator cuff muscle controls a particular movement of the shoulder joint which therapists need to evaluate. For example, is the pain or restriction evident when performing internal rotation (subscapularis), external rotation (teres minor and infraspinatus), and abduction (supraspinatus). To make your treatment more specific and effective, you have to identify which rotator cuff muscle is causing the problem, enabling you to resolve the shoulder problem more quickly. After observation and palpation we need to assess the active and passive movement of the joint. Then the real fun starts by performing specific muscles tests to isolate the culprit. Learning this skill is vital for the successful remedial/sports massage therapist.

Once you have found out which muscle or muscles are compromised you can apply your knowledge and techniques into practice using a variety of skills such myofascial release, trigger point therapy, neuromuscular technique, muscle energy technique, soft tissue release, proprioceptive neuromuscular facilitation and other muscle energy techniques.

After treatment the therapist must not forget to re-test. This shows whether your treatment has had any effect - hopefully positive, but there’s always the chance that things may have got worse. Is there pain free movement? Has the range of movement increased? Follow up treatments need probably be suggested but you will see that your client will leave your clinic feeling confident in their therapist’s skills and knowledge.

The Perils of Underinsurance

We’ve all heard the stories - insurers who won’t pay out when you’ve had a claim; but have you ever considered that for buildings and contents claims it could be the individual who was responsible or that you could be equally at risk? In buildings and content claims the insurer will proportionately reduce the level of settlement if you don’t have sufficient cover in place. For those who operate under the mistaken belief that ‘OK, my goods are worth £20,000, but I’m unlikely to have a claim for any more than £10,000, I’ll just insure for that value as the premiums are a lot less’, they could find any payments significantly reduced due to underinsurance in the event of a claim.


Underinsurance is defined as “Inadequate insurance coverage by the holder of a policy.” Balens claims team have, over the last few years, seen an increase in the amount of claims that have been proportionally reduced due to underinsurance. Whilst insuring your contents and/or buildings, you need to tell the insurer the total value of the items you want to be insured; this is called the “sum insured”. This information is used to assess the risk that the insurers are taking on and therefore the level of premium you will need to pay. If you give a false account of the sum insured, you put yourself in danger of underinsurance in the event of a claim. Where your policy wording highlights an "average clause" in relation to claims and the sum insured is insufficient, only a proportion of your  claim could be paid.

The following example will hopefully give clarity: An individual insures their contents for the sum of £30,000, but find in a claim situation that they need to claim for £35,000. If the insurance loss adjuster assesses the value of the contents at the time when the incident occurred to have been £50,000 the value of the settlement would be reduced to only £20,900 when including a £100 excess. This is £14,100 less than the value of their losses! 

This is calculated as follows:

£35,000 (claim) x £30,000 (Sum insured) 
-----------------------------------------------                  =
£21,000 - £100 (excess) - £20,900 (proposed insured)

£50,000 (Value at Risk or assessment of the
actual contents value at the time of the claim)

It can be difficult to estimate the value of your contents or the cost of rebuilding a property, which is why a majority of insurers will offer package policies allowing you some room for manoeuvre. However business owners should be able to gain an idea of their contents cost from financial records. It is wise to be as accurate as possible and to ensure that the sums insured are updated should your contents costs increase, for example if you purchase new equipment, as it will be your responsibility to declare and regularly review your sums insured.  Hopefully this brief article has made you aware of the cause and effects of underinsurance, however should you have any question or are in any doubt with regards to your own level of cover, please do check with your Insurance Broker, they are there to help.

Sodium Bicarbonate - Nature’s Unique First Aid Remedy

If you live in the States, you can get fifty pounds for fifty dollars of high-grade medicine that you can drink or put in your bathtub for the quickest, safest and certainly least expensive anticancer treatment available anywhere for any price.

For ten dollars or less one has a nothing-to-lose-everything-to-gain-cancer-treatment. Sodium bicarbonate is a world-class anti-fungicide, but that is but one of the reasons why baking soda is such an effective, safe and rapid-acting anti-tumour agent. It is such a good and necessary cancer medicine that oncologists have secretly been using it for decades along with chemotherapy, fooling themselves in the process because they think they are using it to protect their patients from the deadly chemo chemicals.

Emergency room and intensive care medicine would be severely handicapped if there was no sodium bicarbonate, and already very dangerous chemotherapy would be even more deadly without bicarbonate used as a buffer when the chemical chemo poisons are pumped in. If you want to see how fast a person can hit the floor during chemotherapy just forget to mix in the bicarbonate and get out your stopwatch!

Sodium bicarbonate is used prior to, during and after application of chemotherapy. Since the very beginning sodium bicarbonate has been used with the premier chemotherapy agent made from mustard gas. Mechlorethamine also known as chlormethine, mustine, nitrogen mustard and HN2, and sold under the brand name of Mustargen, was the prototype anticancer chemotherapeutic drug. Use of mechlorethamine gave birth to the field of anticancer chemotherapy. Without baking soda, orthodox oncology would never have been able to establish itself for all their patients would probably have died too quickly.

You will also be given fluids via IV and a drug called mesna with your cyclophosphamide to help prevent bladder irritation. Sodium bicarbonate will be given to you - usually as a drip - before and during your methotrexate treatment, to help protect your kidneys. These chemo drugs are an analogue of mustard gas and were derived from chemical warfare research.

    Is Juicing Really Good For You?

    Sunlight Research Forum

    Dutch scientists have shown how UV radiation and vitamin D supplementation in mice affect colorectal cancer growth. As high vitamin D and ambient UV levels are known to be associated with lower colorectal cancer mortality, a group of scientists from the University of Leiden, Netherlands investigated in mouse experiments whether this could be a causal relationship. The tumour load was significantly reduced in both the vitamin D-supplemented and the UV-exposed group compared to the control group. Only UV exposure slowed down malignant progression.

    Scientist at the University of East Anglia have shown how the main psychoactive ingredient in cannabis could reduce tumour growth in cancer patients.     Newborn Parents Pay High Price For Interrupted Sleep

    Most new parents report feeling more exhausted in the morning than when they went to bed the night before. Now, in the first study of its kind, Professor Avi Sadeh and a team of researchers from Tel Aviv University’s School of Psychological Sciences explained why interrupted sleep can be as physically detrimental as no sleep at all. In the study, published in the journal Sleep Medicine, Professor Sadeh and his colleagues establish a causal link between interrupted sleep patterns and compromised cognitive abilities, shortened attention spans and negative moods.
